In order for our office to work for you as efficiently as possible, it is necessary for you to read, complete, and sign our office forms. The visit process would be greatly facilitated if, ahead of your visit, you can fax (301-762-7382) the following:

1. Copy of your insurance card (s), to allow insurance verification and benefits, co-pays, deductibles, indicating which one is primary if there is more than one insurance.
2. A completed registration form, available on this website, under “Patient Forms”
3. If your insurance requires a referral to see a specialist, you need to obtain it from your primary care physician, and you may ask that it be faxed it to us at (301)-762-7382

Preparing for your office visit:

Please bring with you:

• Your insurance card.
• Your driver’s license.
• The referral from your referring physician (unless it was faxed to our office).
• A list of current prescription or over-the-counter medications you are taking.
• Names of medications which have caused allergic reactions in the past, if any.
• Names of medications and OTC remedies that have been used to treat the condition for which you are visiting the doctor
• Pertinent information about your medical and surgical history.

We strive to honor all appointment times, while giving every patient the time needed to have questions answered fully by the doctor. Because we cannot predict ahead of time the needs of a particular patient, it is possible at times that a patient might have to wait while someone else's needs are being met.
